The Medical History Questionnaire (MHQ) is a requirement for all personnel who are involved in animal research activities including those with direct animal care and use responsibilities, those who access vivarium spaces, those who handle animal tissues, and all PIs and faculty sponsors of ARC protocols. The MHQ is reviewed by medical professionals in Occupational Health (OH) and the Arthur Ashe Student Health & Wellness Center (Ashe), who will ensure proper medical surveillance and care based on risk and medical history.

Effective August 20, 2024, if you work exclusively with rodents, rabbits, dogs, cats, reptiles, or fish (the “All other animals” category), the MHQ renewal requirement will change from annual to every 3 years. All other users will continue to submit a new MHQ annually.
Note: Any MHQ approved prior to August 20 will be approved for one year regardless of the category selected. 

Fill out a new MHQ if you begin work with a new species, have significant changes in your health, or experience worsening of an ongoing medical condition. You are required and fully responsible to update your health information and/or risk category regardless of your MHQ expiration date.
